Each year, we are required to get the permission from a parent or guardian for children to attend and participate in
You may download and print the form now, so you can fill it out at home. We need medical information as well as emergency contacts. On the second page is a covenant of conduct we ask all AFO participants to agree to and sign.
Please have your child bring the completed form to their initial AFO event this year. Only one form is required per program year.
