Parental Consent and Liability Release Form
When children and teenagers participate in Aldersgate activities, the church needs to have a completed and signed Parental Consent and Liability Release form on file for each child and teen. The form is update each year, so we ask that you download and bring a completed form to the first event your child or teen attends in the fall.
